The rotator cuff is a commonly injured area. These can range from minor strains and inflammation to frank tears.
It is an envelope of four tendons surrounding the ball and socket joint of the shoulder. (A tendon is a strong, but flexible tissue which connects muscles to bone). The rotator cuff stabilizes the ball of the shoulder into its socket throughout its tremendous range of movement. The rotator cuff is critically important for shoulder function. Every time you move your shoulder, you are using your rotator cuff to stabilize and help move the joint.
The most common symptoms of a rotator cuff tear are:
Broadly speaking, rotator cuff tears are of two types. They can occur due to age related wear and tear (called “degenerative“) or can follow an injury (called “traumatic”).
Traumatic rotator cuff injuries can occur after a single major injury (like a hard fall onto the shoulder, during a workout, or in an accident)., or due to repeated minor injuries. They occur most often in people who repeatedly perform overhead motions in their jobs or sports. Examples include painters, carpenters, and people who play badminton, tennis or in cricket bowlers and weightlifters.
A thorough clinical assessment by your shoulder surgeon is critical. It may reveal specific sources of the pain, and weakness of specific muscles of the rotator cuff. A thorough clinical examination will also reveal the presence of other associated problems with your shoulder (e.g., capsulitis, shoulder arthritis, etc).
Although X-rays do not visualise the rotator cuff, they reveal any associated bony injuries or arthritis of the shoulder. A well performed ultrasound scan, or an MRI scan usually reveals the location and size of the tear.
The treatment depends on multiple factors.
Broadly speaking, many people recover from rotator cuff disease with physiotherapy exercises, rest, ice and painkillers.
Most of the cases respond well to Physiotherapy. A Comprehensive assessment is completed by a qualified physiotherapist and a specific protocol is designed.
Physiotherapy management includes Pain Management, Manual Therapy, Soft Tissue Mobilizations, Therapeutic exercises and stretching exercises. Patient is discharged with a Home Exercise Program
Physiotherapy improves flexibility and strength of the muscles surrounding the shoulder joint. Your surgeon may sometimes recommend a steroid injection into your shoulder to temporarily reduce the pain. These are usually preferred in the elderly. They need to be used cautiously, as they can cause the tendon to weaken further. When rotator cuff tears may occur as a result of a single injury Orthopedic care should be sought as soon as possible. Complete rotator cuff tears, and those which do not improve with physiotherapy may require surgical repair to prevent the tear from worsening and causing arthritis of the shoulder. Many different types of operations are available for rotator cuff injuries.
Arthroscopic tendon repair : In this procedure, surgeons insert a tiny camera (arthroscope) and tools through tiny cuts in the skin and reattach the torn tendon to the bone. Your surgeon can inspect the entire shoulder joint and repair any other problems that it may have.
Open tendon repair : In some situations, an open tendon repair may be a better option. In these types of surgeries, your surgeon works through a larger incision to reattach the damaged tendon to the bone.

Tendon transfer : If the torn tendon is too damaged to be reattached to the arm bone, surgeons may decide to use a nearby tendon as a replacement.
Shoulder replacement : Neglected rotator cuff injuries may cause arthritis of the shoulder. When this happens, the best option to relieve your pain and regain movement is shoulder joint replacement surgery. A special type of replacement called Reverse Shoulder Replacement is the gold standard in treating arthritis caused by chronic rotator cuff tears.
