Myofascial Syndromes

Myofascial pain syndrome is a chronic pain condition affecting the musculoskeletal system. Most people experience muscle pain at some time that typically resolves on its own after a few weeks. But for some people, muscle pain persists. In people with myofascial pain syndrome (MPS), sensitive spots are known as trigger points. These areas develop in the taut, ropey bands of the muscles (the fascia). When pressure is applied to these trigger points there is pain (called referred pain) in a different part of the body.

Trigger Point Pain

A Trigger Point (TrP) is a hyperirritable spot, a palpable nodule in the taut bands of the skeletal muscles' fascia. Direct compression or muscle contraction can elicit jump sign, local tenderness, local twitch response and referred pain which usually responds with a pain pattern distant from the spot. Jump sign is the characteristic behavioral response to pressure on a TrP. Individuals are frequently startled by the intense pain. They wince or cry out with a response seemingly out of proportion to the amount of pressure exerted by the examining fingers.

Osteoarthritis

Osteoarthritis is the most common form of arthritis, affecting millions of people worldwide. It occurs when the protective cartilage that cushions the ends of the bones wears down over time. Although osteoarthritis can damage any joint, the disorder most commonly affects joints in your hands, knees, hips and spine. Osteoarthritis symptoms can usually be managed, although the damage to joints can't be reversed. Staying active, maintaining a healthy weight and receiving certain treatments might slow progression of the disease and help improve pain and joint function.

Rheumatoid Arthritis

Rheumatoid arthritis is a chronic inflammatory disorder that can affect more than just your joints. In some people, the condition can damage a wide variety of body systems, including the skin, eyes, lungs, heart and blood vessels. An autoimmune disorder, rheumatoid arthritis occurs when your immune system mistakenly attacks your own body's tissues. Unlike the wear-and-tear damage of osteoarthritis, rheumatoid arthritis affects the lining of your joints, causing a painful swelling that can eventually result in bone erosion and joint deformity.

Impingement Syndrome

Shoulder impingement syndrome is the result of a vicious cycle of rubbing of the rotator cuff between your humerus and top outer edge of your shoulder. The rubbing leads to more swelling and further narrowing of the space, which result in pain and irritation. Treatments include rest, ice, anti-inflammatory medications, physical therapy, cortisone injections and surgery.

Cervical Spondylosis

Cervical spondylosis is a general term for age-related wear and tear affecting the spinal disks in your neck. As the disks dehydrate and shrink, signs of osteoarthritis develop, including bony projections along the edges of bones (bone spurs). Cervical spondylosis is very common and worsens with age. More than 85 percent of people older than age 60 are affected by cervical spondylosis. Most people experience no symptoms from these problems. When symptoms do occur, nonsurgical treatments often are effective. For most people, cervical spondylosis causes no symptoms. When symptoms do occur, they typically include pain and stiffness in the neck.

Lumbar Spondylosis

Lumbar spondylosis is a chronic, non-inflammatory disease caused by degeneration of lumbar disc and or facet joints. It can be described as all degenerative conditions affecting discs, vertebral bodies, and associated joints of lumbar vertebrae. The disease is said to be progressive and irreversible in older patients. Lumbar region is often the most affected because of the exposure to mechanical stress.

Disc Prolapse

A prolapsed (herniated) disc occurs when the outer fibers of the intervertebral disc are injured, and the soft material known as the nucleus pulposus, ruptures out of its enclosed space. The prolapsed disc or ruptured disc material can enter the spinal canal, squashing the spinal cord, but more frequently the spinal nerves. Herniated discs rarely occur in children, and are most common in young and middle-aged adults. A herniation may develop suddenly, or gradually over weeks or months.

Spondylolisthesis

Spondylolisthesis is a spinal condition that causes lower back pain. It occurs when one of your vertebrae, the bones of your spine, slips out of place onto the vertebra below it. Most of the time, nonsurgical treatment can relieve your symptoms. If you have severe spondylolisthesis, surgery is successful in most cases.

Low Back Pain

Acute back pain can last anywhere from a few days to a few weeks, while chronic back pain is pain that lasts longer than three months. Low back pain is more likely to occur in individuals between the ages of 30 and 50. This is partly due to the changes that occur in the body with aging. As you grow older, there’s a reduction in the fluid content between the vertebrae in the spine. This means discs in the spine experience irritation more easily. You also lose some muscle tone, which makes the back more prone to injury. This is why strengthening your back muscles and using good body mechanics are helpful in preventing low back pain.

Sciatica

Sciatica refers to pain that radiates along the path of the sciatic nerve, which branches from your lower back through your hips and buttocks and down each leg. Typically, sciatica affects only one side of your body. Sciatica most commonly occurs when a herniated disk, bone spur on the spine or narrowing of the spine (spinal stenosis) compresses part of the nerve. This causes inflammation, pain and often some numbness in the affected leg. Although the pain associated with sciatica can be severe, most cases resolve with non-operative treatments in a few weeks. People who have severe sciatica that's associated with significant leg weakness or bowel or bladder changes might be candidates for surgery.

Kyphosis

Kyphosis is an exaggerated, forward rounding of the back. It can occur at any age but is most common in older women. Age-related kyphosis is often due to weakness in the spinal bones that causes them to compress or crack. Other types of kyphosis can appear in infants or teens due to malformation of the spine or wedging of the spinal bones over time. Mild kyphosis causes few problems. Severe kyphosis can cause pain and be disfiguring. Treatment for kyphosis depends on your age, and the cause and effects of the curvature.

Frozen Shoulder

Frozen shoulder, also known as adhesive capsulitis, is a condition characterized by stiffness and pain in your shoulder joint. Signs and symptoms typically begin gradually, worsen over time and then resolve, usually within one to three years. Your risk of developing a frozen shoulder increases if you're recovering from a medical condition or procedure that prevents you from moving your arm — such as a stroke or a mastectomy. Treatment for frozen shoulder involves range-of-motion exercises and, sometimes, corticosteroids and numbing medications injected into the joint capsule. In a small percentage of cases, arthroscopic surgery may be indicated to loosen the joint capsule so that it can move more freely.

Plantar Fasciitis

Plantar fasciitis (PLAN-tur fas-e-I-tis) is one of the most common causes of heel pain. It involves inflammation of a thick band of tissue that runs across the bottom of your foot and connects your heel bone to your toes (plantar fascia). Plantar fasciitis commonly causes stabbing pain that usually occurs with your first steps in the morning. As you get up and move, the pain normally decreases, but it might return after long periods of standing or when you stand up after sitting. Plantar fasciitis is more common in runners. People who are overweight and those who wear shoes with inadequate support also have an increased risk of plantar fasciitis.

Sports Injuries

Sports injuries occur during exercise or while participating in a sport. Children are particularly at risk for these types of injuries, but adults can get them, too. Different sports injuries produce different symptoms and complications. The most common types of sports injuries include: sprains, strains, knee injuries, achilles tendon rupture, fractures, dislocations and rotator cuff injury.

Knee Pain

Knee pain is a common complaint that affects people of all ages. Knee pain may be the result of an injury, such as a ruptured ligament or torn cartilage. Medical conditions — including arthritis, gout and infections — also can cause knee pain. Many types of minor knee pain respond well to self-care measures. Physical therapy and knee braces also can help relieve pain. In some cases, however, your knee may require surgical repair.

ACL, PCL

The anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) are two major ligaments in the knee that work together to provide stability. They are also common sites of serious tears, particularly in athletes. Although ACL and PCL injuries may initially appear to have similar symptoms such as knee instability and pain, the ligaments have unique characteristics that make them differ in terms of who is affected, extent of injury, and treatment guidelines.

Meniscus Tear

A torn meniscus is one of the most common knee injuries. Any activity that causes you to forcefully twist or rotate your knee, especially when putting your full weight on it, can lead to a torn meniscus. Each of your knees has two C-shaped pieces of cartilage that act like a cushion between your shinbone and your thigh bone. A torn meniscus causes pain, swelling and stiffness. You also might feel a block to knee motion and have trouble extending your knee fully. Conservative treatment — such as rest, ice and medication — is sometimes enough to relieve the pain of a torn meniscus and give the injury time to heal on its own. In other cases, however, a torn meniscus requires surgery.

Post Op Rehab

Rehabilitation is the post-surgical program of re-establishing joint motion, muscle strength around the joint and finally joint function. It is very important to understand that rehabilitation is a long process. While the surgery is performed within hours, the rehabilitation needs many months, potentially up to a year. The difference between the length of the surgery and the time needed for rehabilitation is huge, and patients should have that clearly in mind. When the patient undergoes cartilage surgery of the knee, ankle or shoulder, for example, by whatever method, they should be aware that they need a long recovery period.

All Joint Pains

Joints are the parts of your body where your bones meet. Joints allow the bones of your skeleton to move. Joints include: shoulders, hips, elbows and knees. Joint pain refers to discomfort, aches, and soreness in any of the body’s joints. Joint pain is a common complaint. It doesn’t typically require a hospital visit. Sometimes, joint pain is the result of an illness or injury. Arthritis is also a common cause of joint pain. However, it can also be due to other conditions or factors.

IT Band Syndrome. Scoliosis

The iliotibial band is a thick band of fibrous tissue that runs down the outside of your leg and helps provide stability to the outside of the knee joint. IT band syndrome is when this band becomes inflamed. Moving your knee becomes painful; it worsens with continued movement, and it resolves with rest. The cause is usually a sudden increase in activity. Runners who increase their mileage too fast often develop iliotibial band syndrome, as do individuals with biomechanical problems with their gait such overpronation, leg length discrepancies, or individuals who are bow-legged, or those who have dominant pelvic rotation.

Rotator Cuff Tears

A partial or complete rotator cuff tear makes it difficult to raise and move your arm. You may have shoulder pain and arm weakness. Rotator cuff injuries are common, especially as you get older. Rest, pain relievers and physical therapy can help. Some people need surgery to reattach a torn rotator cuff.

Carpal Tunnel Syndrome

Carpal tunnel syndrome is the compression of the median nerve as it passes into the hand. The median nerve is located on the palm side of your hand (also called the carpal tunnel). The median nerve provides sensation (ability to feel) to your thumb, index finger, long finger, and part of the ring finger. It supplies the impulse to the muscle going to the thumb. Carpal tunnel syndrome can occur in one or both of your hands. Swelling inside your wrist causes compression in carpal tunnel syndrome. It can lead to numbness, weakness, and tingling on the side of your hand near the thumb.

Dequervains Syndrome

De Quervain's tenosynovitis (dih-kwer-VAINS ten-oh-nine-oh-VIE-tis) is a painful condition affecting the tendons on the thumb side of your wrist. If you have de Quervain's tenosynovitis, it will probably hurt when you turn your wrist, grasp anything or make a fist. Although the exact cause of de Quervain's tenosynovitis isn't known, any activity that relies on repetitive hand or wrist movement — such as working in the garden, playing golf or racket sports, or lifting your baby — can make it worse.

Tennis Elbow

Tennis elbow (lateral epicondylitis) is a painful condition that occurs when tendons in your elbow are overloaded, usually by repetitive motions of the wrist and arm. Despite its name, athletes aren't the only people who develop tennis elbow. People whose jobs feature the types of motions that can lead to tennis elbow include plumbers, painters, carpenters and butchers. The pain of tennis elbow occurs primarily where the tendons of your forearm muscles attach to a bony bump on the outside of your elbow. Pain can also spread into your forearm and wrist.

Golfers Elbow

Golfer's elbow is a condition that causes pain where the tendons of your forearm muscles attach to the bony bump on the inside of your elbow. The pain might spread into your forearm and wrist. Golfer's elbow is similar to tennis elbow, which occurs on the outside of the elbow. It's not limited to golfers. Tennis players and others who repeatedly use their wrists or clench their fingers also can develop golfer's elbows. The pain of a golfer's elbow doesn't have to keep you off the course or away from your favorite activities. Rest and appropriate treatment can get you back into the swing of things.

TMJ pain, Neck Pain, Tra

The temporomandibular (tem-puh-roe-man-DIB-u-lur) joint (TMJ) acts like a sliding hinge, connecting your jawbone to your skull. You have one joint on each side of your jaw. TMJ disorders — a type of temporomandibular disorder or TMD — can cause pain in your jaw joint and in the muscles that control jaw movement. The exact cause of a person's TMJ disorder is often difficult to determine. Your pain may be due to a combination of factors, such as genetics, arthritis or jaw injury. Some people who have jaw pain also tend to clench or grind their teeth (bruxism), although many people habitually clench or grind their teeth and never develop TMJ disorders.

Ligament Sprains

A sprain is a stretching or tearing of ligaments — the tough bands of fibrous tissue that connect two bones together in your joints. The most common location for a sprain is in your ankle. Initial treatment includes rest, ice, compression and elevation. Mild sprains can be successfully treated at home. Severe sprains sometimes require surgery to repair torn ligaments. The difference between a sprain and a strain is that a sprain injures the bands of tissue that connect two bones together, while a strain involves an injury to a muscle or to the band of tissue that attaches a muscle to a bone.

Muscle Strains

A muscle strain is an injury to a muscle or a tendon — the fibrous tissue that connects muscles to bones. Minor injuries may only overstretch a muscle or tendon, while more severe injuries may involve partial or complete tears in these tissues. Sometimes called pulled muscles, strains commonly occur in the lower back and in the muscles at the back of the thigh (hamstrings). The difference between a strain and a sprain is that a strain involves an injury to a muscle or to the band of tissue that attaches a muscle to a bone, while a sprain injures the bands of tissue that connect two bones together. Initial treatment includes rest, ice, compression and elevation. Mild strains can be successfully treated at home. Severe strains sometimes require surgical repair.

Tendinopathy

Tendinopathy, also called tendinosis, refers to the breakdown of collagen in a tendon. This causes burning pain in addition to reduced flexibility and range of motion. While tendinopathy can affect any tendon, it's more common in the: Achilles tendon.
